This notice acknowledges that the customer,

desires MVI Solutions to give customer file  transfer protocol (FTP) codes for the Web site

___________________________________________________ (Web site domain name). 

In authorizing the release of this sensitive information, the customer acknowledges the following:

  1. Customer will take all reasonable care to guard the security 
    of the codes, and will guard the codes with at least the same 
    degree of care as it would guard any of its own proprietary 
    information.

  2. Customer releases MVI Solutions from the responsibility of 
    ensuring the proper functioning of the website files. Any work 
    required to repair broken website files will be performed by 
    MVI Solutions at normal rates.

  3. Customer will not use codes to attempt to damage or 
    disrupt in any way the website systems of MVI Solutions.

  4. Customer will not use the codes to post illegal material, 
    or material of an offensive nature, on customer’s website.

  5. MVI Solutions’s sole responsibility is to provide access codes. 
    MVI Solutions personnel is not responsible for training the 
    customer how to make website changes, or training customer 
    how to use FTP software.

  6. MVI Solutions may revoke FTP access at any time for any 
    reason, at its sole discretion.
